Guidelines and forms for use by General Members seeking accreditation.

REACCREDITATION

Re-accreditation occurs on 1 July each year. Accredited members are required to submit:

  1. Supervision and Clinical Hours Log / Mentorship and Teaching Hours Log
  2. CPD Activity Log
  3. Certificate of Insurance
  4. Evidence of Registration with Approved Professional Association (Psychosexual Therapists only)
  5. Current National Police Check and/or Working With Children Check.

Supervision and Clinical Hours / Teaching and Mentorship Hours

Use to declare supervision and clinical hours/teaching and mentorship hours undertaken during the period.

SUPERVISOR/MENTOR APPROVAL

Use the following forms if your supervisor/mentor is not an accredited SAS supervisor/mentor to ensure your supervision/mentorship hours meet requirements.

Use the following form if you are an accredited member seeking to be listed as a SAS Supervisor/Mentor:

Continuing Professional Development (CPD)

Use to log your CPD hours for the period. Refer to the SAS CPD Guidelines and Example document to ensure your CPD meets requirements.

LEAVE OF ABSENCE (LOA)

For use by General and Accredited Members seeking a Leave of Absence from their membership/accreditation status.

RETIREMENT POLICY

For use by General and Accredited Members retiring from teaching/practice.

MEMBERSHIP REFEREE REPORT FORM

For use by those seeking SAS Membership.
